The demands of chronic conditions and disabilities challenge children and their families
to manage their chronic condition and disability on a daily basis as children grow
and develop. Children and adolescents are challenged to learn self-management skills
and are vulnerable to developing stress created by the interplay between their developmental
needs and the demands of managing the chronic condition and disability (
Christian, 2016a
,
Christian, 2016b
,
Christian, 2019
). As children with chronic conditions and disabilities become adolescents, they gain
independence in self-management from their parents. However, the need to prepare for
transitions from pediatric to adult healthcare, challenges adolescents' newly developed
independence in self-management creating additional stress. Developing resilience
and support may serve as protective factors for coping with stress associated with
managing the chronic condition and disability. However, preparing for healthcare transitions
creates additional caregiving demands and stress for youth and their families. Innovative
nursing interventions may enhance coping ability and resilience among youth managing
their chronic conditions and disabilities while reducing stress (
Christian, 2022
). To meet the challenge of healthcare transition readiness for successful transitions
from pediatric to adult healthcare, guidelines and standards of pediatric nursing
practice were developed to address the needs of adolescents and young adults with
chronic conditions and disabilities (
Betz, 2017
